Q: I met with a counselor a long time ago, how do I start up again?
Just give us a call at (801) 863-8747 and let us know that you have been in before. We will set up an appointment to meet with you again. If you have been here in the past 7 years, we should still have your information and documentation on file.
Q: I don't have any documentation, but suspect that I have a disability - what do I do?

To schedule an appointment please call Accessibility Services at (801) 863-8747 or come by our office in WB 146.
Q: How do I get my letters?
Once you have spoken with your counselor and submitted proper documentation, your counselor will provide letters for you to give to your instructors. You may pick up your letters at the ASD front desk in WB 146.
Q: How do I get note takers?
If your documentation qualifies you for this service, you may have peers in your classes take notes for you. Once this accommodation is approved by your ASD counselor, you must meet with the Manager of Accommodative Services (April Crawford) in WB 146. At that time you will sign a contract and be given a form to take with you to class so that you may identify a note taker.

ASD does not provide tutors as an accommodation, but there are other places on campus you may check with to get tutorial help.
Q: What is the ASD Lab, and can I use it?
Q: I am taking notes for someone in my class, what do I need to do?
First, make sure you know the name of the person you are taking notes for. Then come see the service coordinator (April Crawford) in LC 416 where you will need to sign a contract. You will receive more instructions at that point.
Q: How do I get a handicapped parking tag?
In order to be issued a permanent
disabled parking permit from UVSC, you must first obtain a disabled permit from the State of Utah. To purchase a disabled parking permit, you need to complete a permit form and provide proof of emission testing. If the vehicle is registered in Utah, Salt Lake, Davis, or Weber counties, the registration is proof of emissions testing.
The cost for disabled permits will be as follows:
Permanent: $80 per year
Temporary: Free
If you have a temporary need for a disabled parking permit, you must first obtain a note from a doctor, stating how long you will need the permit. Present this documentation to
parking services, and a temporary permit may be issued.

PROCEDURE FOR SCHEDULING TESTS:

Make sure you know the date that your exam starts.

You MUST schedule your tests 24 hours in advance.

You are REQUIRED to take your test the same time as the rest of your class. The only exception is if your instructor has given you permission to take it at a later date. The PEC will call to verify this change with your instructor.

Call the PEC to set up an appointment. You may call the numbers above or email us to set up an appointment.
IN ORDER TO SCHEDULE, YOU WILL NEED TO KNOW THE FOLLOWING:

Where your test is being administered (i.e. Classroom Tesing Center (CTC), in class).

Who your instructor is.

The class (i.e. CLSS 1000).

The date and time that you would like to schedule.

If you need special accommodations, which are cleared through ASD (i.e. scribe, reader, tape), you will need to let the PEC know so that we can accommodate you.

If you are taking the test later than the rest of your class, you will need to inform us when you schedule your test.
RESCHEDULING AND CANCELING OF APPOINTMENTS:

A 24 hour notice is REQUIRED if you need to cancel or reschedule an appointment.

Two no-shows will result in a mandatory meeting with your ASD counselor. You will need clearance from your counselor before you can take another test in the PEC.
SCHEDULING FINAL EXAMS:

All final exams taken in the PEC should be scheduled as soon as possible.

Finals need to be scheduled during the same time as your classmates.

The final schedule for your class may be found in the UVSC Class Schedule Book.
Attendance/Participation
You are responsible to attend class, complete all assigned work, participate in class, and notify your instructors of any special needs in a timely manner. If you fail to follow through with these responsibilities, your instructor may choose to submit a progress report to your ASD counselor. Upon receipt of a progress report, your ASD counselor will require a conference with you to evaluate the use of accommodative services. A minimum of 90% class attendance is expected. Services may be suspended or terminated for excessive absences.
Student Accommodations Letter
At the beginning of each semester, you must make an appointment with your ASD counselor. At this time, any necessary changes in your Student Accommodations Letter will be made. You are required to give a Student Accommodations Letter to each of your instructors when you are receiving accommodations that involve any action on the teacher's part. Attempts at falsification or unauthorized use of Student Accommodations Letters from the ASD office will be treated as criminal acts of fraud.
* If difficulties arise with your instructor concerning your accommodations, we encourage you to self-advocate by speaking with your instructor and contacting your ASD counselor.
Q: What do I need to put on my syllabus regarding Accessibility Services or the ADA?

If you have any disability that may impair your ability to successfully complete this course, please contact the Accessibility Services Department (WB-146; 863-8747). Academic Accommodations are granted for all students who have qualified documented disabilities. Services are coordinated with the student and instructor by the Accessibility Services Department.
Q: What am I required to do when I have a student with a disability in my class?
Students who request accommodations are required to submit a letter to you stating which accommodations they qualify for. These letters should come from an ASD counselor and be in the following format:

When you receive one of these letters, you are obligated to provide the student with the accommodations listed therein. The student's identity, disability and limitations are to remain confidential at all times. If you have questions about how to provide any of the accommodations, you may contact the ASD counselor whose name appears on the letter.
If you receive a letter, this means that the student has met with an ASD counselor and submitted the proper documentation to qualify for the services according to the law (ADA).
Q: How can I help a student with a disability succeed?
It is important that as an instructor, you are sensitive to the needs of the student with the disability. Their accommodations and limitations should be kept confidential. You should also remember to treat them as any other student with the exception of their accommodations. Often, making yourself available to the student and establishing a foundation of trust from the beginning can be instrumental in helping the student succeed.
Q: How do I handle tests in the PEC?
The PEC will take care of the testing accommodations requested in the letter you receive from the student's ASD counselor. They will have a copy of each letter on file and provide the needed services. As an instructor it is your responsibility to place your test in the PEC. Please also see the following guidelines:

Option One -
- Please deliver the test to the Proctored Exam Center at least 24 hours prior to the start of the test.
- If you prefer, you may bring down ALL tests for the entire semester.
- You will be required to fill out a green test clarification slip, which specifies which testing aids are allowed.
- After the test period is terminated, you or a designee from your department may pick up the test from the PEC. Tests completed in the PEC will NOT, for any reason, be released to ASD students.
Option Two -
In some instances, and upon student agreement, you or a departmental designee may administer tests in a quiet, uninterrupted environment, utilizing the accommodations specified in your faculty letter.
Q: What do I do for students that need note takers?
Students who qualify for note takers will give you a letter from their ASD counselor. The student then needs to meet with the service coordinator (April Crawford) in LC 416. At that time they will sign a contract and pick up a yellow paper to give to you. Once the student gives you the paper, you should read it to your class and send it around for people to volunteer. After class, please collect the paper and give it to the ASD student. If the student does not provide you with the yellow paper, you are not required to provide this service. If no one in your class volunteers the first time, please restate the importance of the service and send the sheet around again.
Q: What do I do for students who need text on CD?
This is an accommodation provided by ASD. Please refer the student to their ASD counselor who will assist them in setting up the service.
Q: Who do I contact if I have a problem with the student's accommodations?
If you have any questions about the accommodations stated in the
letter you receive, or if you feel that the accommodation would alter the essential functions of the course, please contact the ASD counselor listed on that letter at 863-8747.
Q: What are my obligations regarding confidentiality and the students' disabilities?
When a student discloses that he or she has a disability, it is your responsibility to make a good faith effort to protect the student's right to privacy regarding his or her disability status. When dealing with disability issues:
- Be cautious not to discuss the student's disability in front of others;
- Maintain the student's right to privacy by not disclosing to others that the student has a disability;
- Be available to meet with students to discuss accommodations and services that they may qualify for;
- Remember that it is not appropriate to ask students what their disabilities are. However, on a "need to know basis," you may ask the ASD counselor to share the identified functional limitations;
