Must have government issued PHOTO ID:

Valid Driver's License
State ID
Passport
Military ID

If government issued photo ID is not available, 3 forms of ID one of which must include photo ID (ie: yearbook, birth certificate, social security card, parent's driver's license if name is on birth certificate, billing statements, bank statements, church records, Nonresident Alien Registration, Police Pistol Permit with Photo, Indian blood certificate, Court of Record documents.)

Note: The student can have these items faxed to Campus Connection at no charge as long as the items are verifiable and legible.
