LOST/STALE DATED TUITION REFUND CHECKS

 

Lost Refund Checks:
If you have not received your tuition refund check in the mail within 21 days from the date it was issued, please contact the Business Services Department at (801)863-7496 or (801)863-8299.

Stale Dated refund Checks:
Refund checks must be cashed within 90 days of the issue date. Stale dated checks must be returned to the Business Services Department to be re-issued.
