Entrance Loan Counseling
TERMS AND CONDITIONS
Loan Counseling for FALL of any given school year is accepted and valid JUNE 1 and afterwards. DO NOT TAKE LOAN COUNSELING FOR FALL SEMESTER BEFORE JUNE 1 as it will not count towards completion of this requirement before that date.
Upon completion of the Loan Counseling test section you will be presented with a form that Must Be Completely Filled Out. Forms that are not completely filled out with all required Names, Addresses, Telephone Numbers, etc will Not Be Accepted and Loan Counseling will need to be re-submitted. Please have the information needed before taking the online loan counseling for Driver's License, Spouse, Parent, Employer, Next of Kin and two complete references that do not have any of the same information as Student, Parent, Spouse, Next-of-Kin, as well as your decided Major at UVSC. All fields are required.
Allow three (3) working days for your information to be transferred from the Mapping-Your-Future database to the UVSC system. If after this time your status still shows that you need to complete Loan Counseling, contact the UVSC Financial Aid office to inquire about the status of your submitted Loan Counseling form.
1) I agree that I am accepting all the loans and loan amounts for all semesters specified on my Financial Aid Award Notification. If you do not want all or part of the award, you must notify the Financial Aid Office before accepting these terms and conditions.
2) My loan is subject to revision or cancellation if conditions, costs, or Federal/State regulations change, or if an error is made in calculation of my eligibility for this award whether by the Federal Processor or the Institution.
To complete loan counseling, please read the following terms and conditions of the Federal Stafford, Perkins or PLUS Loan you are accepting. By clicking on the "I Accept" button, you agree with and will abide by those terms and conditions. PLEASE READ ALL TERMS AND CONDITIONS BEFORE ACCEPTING AND CONTINUING !!
1) I agree that I am accepting all the loans and loan amounts for all semesters specified on my Financial Aid Award Notification. If you do not want all or part of the award, you must notify the Financial Aid Office before accepting these terms and conditions.
2) My loan is subject to revision or cancellation if conditions, costs, or Federal/State regulations change, or if an error is made in calculation of my eligibility for this award whether by the Federal Processor or the Institution.
3) I agree to let the Financial Aid Office know before I completely withdraw from school during the semester after aid has been disbursed to me.
4) I forfeit the balance of my financial aid if I decline or cancel my award, if I fail to enroll, or if I withdraw from or leave school.
5) I must maintain "Satisfactory Academic Progress" as outlined in the Financial Aid Handbook or I could forfeit some or all of my loan.
6) I agree to complete Exit Loan Counseling within 30 days after I stop attending UVSC at least half time or upon graduation from UVSC.
7) I forfeit this award if I fail to accept these terms and conditions by the last date of classes for the specified term/semester.
8) I agree that by failing to accept these terms and conditions within the stated 30 day time frame, my classes may be dropped and a late fee/re-instatement fee may be assessed. It will be my responsibility to pay any fees associated with my classes being purged and understand that I must have my classes re-instated before an award can be re-issued and disbursed.
9) I understand that it is my responsibility to ensure my tuition bill is paid in full. I understand that my Financial Aid award alone may not pay for my entire tuition and fees and that I must pay any difference.
I ACCEPT
