2. Get names of witnesses and information necessary for accident report
   
   • Name of injured person
   • Injured person's address and telephone number
   • Date and time of injury
   • Brief summary of action taken
   • Names and phone numbers  of any witnesses
3. Forward this information to College Police at ext. 5555
