School Forms
Steps to submit the Veterans School Forms via e-mail:
Step 2: Link to the desired School Form (listed above).
Step 3: Enter your information in the form.
(Press the Tab key to navigate between fields and enter your information.)
Step 4: Open your e-mail.
Step 5: Click on the Submit button at the bottom of the form page.
(After you click on Submit, verify the form is attached to an outgoing e-mail.)
Step 6: Send the attached form from your e-mail address to one of the Veterans Advisors.
(Confirm the e-mail was sent in your e-mail system's outbox.)
ALWAYS click on the Reset Form button after you have submitted the form.
Step 7: Check your UVLink e-mail address.
(You will receive a confirmation e-mail from the Veterans Office within a few days of submitting the form. If you do not receive a confirmation e-mail via UVLink from our office, please contact us.)
VA Forms
- Complete the appropriate VA ON-line APPlication (VONAPP) as listed below:
- Reservist/Guard - Chapters 1606 & 1607; Active Duty - Chapter 30
- Dependents - Chapter 35
- VA Form 22-5490 - Application for Survivors' & Dependents Educational Assistance
- VA Form 22-5495 - Application for Change of Program or Place of Training, Survivors' & Dependents
- VA Vocational Rehabilitation - Chapter 31
- Application is at your local VA Regional Office
- Changes are approved through your local VA Regional Office
- NOTE: VA forms may NOT be faxed, the originals must be mailed or delivered to UVSC Veterans Office.
