Request For Change of Program
or Place of Training
Veterans receiving benefits under Chapters
30,
1606, or
1607 must complete and submit
VA Form 22-1995 to the UVSC Veterans Office when submitting Veterans Class Schedule form if changing program (degree, major, concentration) or place of training (institution).
For help in determining a major, Veterans should visit the
Career Counseling Center for analysis of strengths, weaknesses and potential fields of study.

Transfer Credit Evaluation
Evaluation of Transcripts from other institutions must be done by the Transfer Credit Office and/or Veterans Office (if Military).
Transcripts from other institutions attended must be evaluated to determine courses that will transfer toward the Veteran's chosen major. If all transcripts are not available at the time of certification the Veteran may be certified for one Semester only. Certification beyond one term will not be possible until all transcripts have been received and evaluated.
When changing majors, evaluation of the Veteran's transcripts again becomes necessary in determining the courses that apply toward the new educational objective.

Regardless of whether the Veteran has received VA benefits or not, transcripts for prior course work completed must be evaluated. The Veteran is responsible to provide UVSC with an official copy of the transcripts. Veterans are informed of this requirement during the first initial interview. If transcripts are not submitted to the Veterans Office, the Veteran will not be certified for a second semester.

Notice of Change in Student Status
When a student makes changes in their class schedule (add, drop, withdraw from UVSC, deployed, etc.),
the Veterans Office MUST be notified as soon as possible.
If deployed, Veteran will need to submit the UVSC Withdrawal form and copy of deployment orders to the UVSC Veterans Office.
If classes are dropped or withdrawn due to circumstances beyond the Veteran's control (medical, death in the family, etc.), a "Statement in Support of Claim" form can be completed and submitted to the UVSC Veterans Office.
