Orem Fitness Center
We are now taking sign ups for the Orem Fitness Center. These passes are ONLY FOR EMPLOYEES NOT STUDENTS! Fill out the form on the link below and intercampus mail it and payment (checks can be made out to UVU Student Health Services) to Student Health Services Attn: Wellness Programs at mailstop 200. You must be a UVU employee (part-time, full-time, adjunct, etc) to qualify, and spouses and dependent children qualify as well.
Passes will be $12.00 per person (including spouses and dependent children - only children 4 years old and over need a pass) for the year. The address for Orem Fitness Center is 580 W. 165 S. in Orem. Their phone number is 229-7154. All passes will be available to use after November 15th, 2007 and can be renewed in November 2008. We will send the list of people who have signed up over to the Orem Fitness Center every Friday so when you are ready to start using your passes all you will have to do is the following:
*Go to the Orem Fitness Center
*Tell them you purchased a pass through UVU
*Fill out new member paperwork (you will still need to do this even if you are just renewing your membership)
*Pay $2-$3 for your ID card (bring Drivers License, State or School ID)
